APPLICATION FORM
Applications for the 2020 Practice Manager of the Year Awards are now open. If you have been nominated by a colleague to apply or you wish to apply yourself for the 2020 Practice Manager of the Year Award, please complete the Application Form below.
ALL Applicants must complete this form.
In order to complete and submit the application form you will need the following as the application must be completed at the one time. You are unable to save as you go.
- AAPM Membership number
- Practice profile (up to 50 words)
- A short Bio (100 words)
- Responses to the selection criteria: there is a separate response field for each criterion (approx 150-250 words for each)
- Two (2) referee statements supporting your application. These can be from a Principal in the practiceor an independent observer
- One (1) Profile photo (headshot, dimensions - 1200px x 1800px, up to 2MB)
No additional material (e.g. other images, videos, other documents) may be included with the application. This is to ensure a fair and equitable process for all applicants. If selection panels require additional information to assist them in making a decision, all applicants will be contacted with this request.
Please Note: You will NOT be able to save a partially completed form, so you will need to have all the information ready before applying. It is also advisable to make sure you have all your information saved as a word document to avoid losing any content and can re-enter it, should you experience technical difficulties when submitting your application.
