14 September GP incentives increased to enrol DVA clients September 14, 2018 By Department of Veterans Affairs dva 0 The Department of Veterans’ Affairs (DVA) has increased incentive payments for General Practitioners (GPs) to enrol eligible patients in the Coordinated Veterans’ Care (CVC) program. The benefits of enrolling your patients, including reduced hospital admissions, were recently highlighted in an in-depth evaluation of the program. GP clinics receive an initial incentive payment to enrol eligible DVA clients, then quarterly care payments thereafter. The new DVA fee schedule for CVC claims, including an increase in payments, came into effect on 1 July 2018. The CVC program, which was established in 2011, supports GPs to manage chronic conditions for eligible DVA clients who are most at risk of unplanned hospitalisation. It is available to DVA Gold Card holders, including veterans, war widow/widowers and dependents, who are living in the community and have one or more chronic conditions. GPs are able to provide ongoing, comprehensive and coordinated care to eligible Gold Card holders, with the assistance of their nurse coordinator. Reduced hospital admissions Early findings from in-depth analysis of CVC Program data commissioned by DVA this year are showing there are demonstrable benefits for participants enrolled in the CVC program, particularly in the second and third year. There appears to be better health outcomes, better relationships between health professionals and DVA clients and reduced hospital admissions. As part of a new CVC Monitoring Evaluation Framework introduced in 2018, DVA is further expanding the scope of data collected under the program for more comprehensive monitoring and analysis in the future. Simplified process with DVA Toolbox for CVC To further simplify CVC enrolment, management and claiming for GP clinics, DVA has developed and is currently testing a new DVA Toolbox for CVC, which provides a convenient online centre of information and resources. The toolbox includes a CVC Eligibility Checker, Care Plan Builder and Claim Calculator, which creates a claim timetable that can be incorporated into a practice’s reminder system.
